Survivor Family ProgramOur Fallen Warriors have paid the ultimate sacrifice and their Families deserve our respect, gratitude and the very best we can provide. As a part of Survivor Family Program, The Hale Koa is offering The Survivor Family Program applicable to anyone with an ID indicating DB or URW (Unremarried Widow) 50% OFF Room Rates by applicable category. Limited to the first 14 nights of your stay. Survivor outreach Services is designed to embrace and reassure our Survivors that their Army will always be there for them. Eligible Personnel IncludesFamily Members who have suffered a loss of a sponsor in the event of an Active Duty death regardless of cause (combat, accident, suicide, illness, etc.) and also may be uniformed personnel. Eligibility Definition and RequirementsThe Survivors’ Family Program extends to Family Members and sponsored guests of accompanying authorized users. Eligible users are defined as Family Members who have suffered a loss of a sponsor in the event of an Active Duty death regardless of cause (combat, accident, suicide, illness, etc.) and also may be uniformed personnel. Proof of EligibilitySurvivor Family Program participants must show Identification Cards with “DB” (Death Beneficiary) or Unremarried Widow shown on the Sponsorship Status filed or must present a DD1300 for purpose of Army Regulation 215-1, Morale, Welfare, and Recreation Activities and Nonappropriated Fund Instrumentalities, 31 Jul 08. Program DetailsThe benefits of this policy will be extended to Survivor Families of all Services based on appropriate applicable rate category. The users will receive a 50 percent discount from normal room rates by applicable category for a maximum duration of 14 nights. The discount will apply to additional occupants and up to three guest rooms (maximum four rooms). In order for the Survivors’ Family Member discount to be extended, the Survivors’ Family Member must make and confirm reservations and be listed as the primary occupant. The Survivor Program guest using the program can take advantage of this special for one stay within a 365 day period. Please call room reservations at 1-800-367-6027 (CONUS). Eligible personnel must mention that they are a part of the U.S. Survivors' Family Program (The Survivor Family Program is applicable to anyone with an ID indicating DB or URW (Unremarried Widow)). Valid Military Identification Cards with “DB” (Death Beneficiary) or Unremarried Widow shown on the Sponsorship Status filed or a DD1300 must be presented at time of check in.
